¡Hola! Yo soy el Dr. Omar Valderrama👨🏻⚕️. Aquí encontrarás todo lo que tienes que saber sobre los miomas. ¿Qué son? ¿Por qué suceden? ¿Quién tiene más riesgo de desarrollarlos? ¿Qué síntomas produce? ¿Cuál es el tratamiento?.
☑️¿Qué son?
Los miomas, también llamados fibromas o leiomiomas, son tumores no cancerosos (benignos? del músculo uterino. Estos pueden ser muy grandes, del tamaño de un melón o tan pequeños como una uva.
Son muy comunes, por lo menos el 20% de las mujeres mayores de 35 años lo llegan a padecer.
☑️¿Por qué suceden?
No se conoce el motivo o causa exacta, lo que sí se conoce es que hay un componente genético, por ejemplo, si tu mamá padece de miomas tienes una alta posibilidad de padecerlos. También se han identificado un aumento en la estimulación hormonal en los receptores de estrógenos y progesterona.
☑️¿Qué aumenta probabilidad?
Los factores de riesgo son:
✔️Estar entre 30 y 50 años y no haber pasado por la menopausia.
✔️Familiares con antecedentes de miomas.
✔️Afroamericana.
✔️Primer período temprano.
✔️No haber tenido hijos.
✔️Tener sobrepeso u obesidad.
☑️¿Síntomas?
✔️Muchas mujeres son asintomáticas
✔️Sangrado menstrual abundante.
✔️Sangrado entre períodos.
✔️Dolor y presión en la zona pélvica.
✔️Urgencia o más frecuencia.
✔️Incapacidad para tener hijos.
✔️No llevar el embarazo a término.
☑️¿Diagnóstico?
Primero tenemos que sospecharlo por los síntomas que la paciente nos refiere, posteriormente realizamos una exploración física tratando de identificar los miomas. Para confirmar tenemos que hacer pruebas de imagen como ultrasonido, tomografía o resonancia magnética.
☑️¿Tratamiento?
Podemos usar medicamentos hormonales, el inconveniente de esto es su baja efectividad y que una vez interrumpidos tienden a volver los síntomas. También podemos usar el dispositivo intrauterino hormonal para disminuir el sangrado ocasionado por los miomas.
Los tratamientos quirúrgicos pueden ser conservadores o radicales, el conservador sería la miomectomía que consiste en quitar solo en mioma, esto es el tratamiento ideal para mujeres que deseen quedar embarazadas. Para las mujeres que ya no deseen tener hijos la opción ideal es la histerectomía, es el tratamiento definitivo, en cual quitamos el útero y con eso conseguimos la resolución total.
